The form is available in two formats:

• Adobe Acrobat (pdf) - download, print and complete by hand

Microsoft Word - Celebrate Clay 2010 Application Word Document

Right-click the link above to download. Choose either “Save Link As” (Netscape) or “Save Target As” (Internet Explorer).  Save the file to your computer when prompted. Once the download is complete, open the form in Word and save it with a unique file name. Enter information in the document fields as you would in a normal word processing document (press Tab to move between fields). Upon completion, save the file. Finally, either print and mail the form to the address below, or attach it to an e-mail message and send to aparker@reinhold.net
